Community Health Navigation (CHN) Program
Our Community Health Navigation Program provides free healthcare navigation for Austin’s vulnerable Asian subpopulations, including the Burmese, Korean, & Vietnamese communities. Services include:
- Patient advocacy
- Interpretation/translation support
- Health navigation education (seminars, workshops, translated materials & communications, etc.)
- Referrals to local resources
- Vaccination events & Annual Health Fair
- Eligibility & Application Assistance – Our Community Health Workers can help you with the eligibility & application process of several health coverage & other programs
Program Eligibility
Individuals who are eligible to receive assistance must be Travis county residents & able to provide proof of identity, residency, & income. Interested individuals may refer to the contact information below to get in touch with their specific language Community Health Navigator (CHN) directly:
