  • Vietnamese Community Health Navigator

    The Austin Asian Community Health Initiative (AACHI) is seeking a new bilingual Community Health Navigator (CHN) to serve the Vietnamese community in Travis County navigate and access health care and community services.

    Job Duties include:

    Health Navigation
    ● Reducing barriers that keep patients from accessing healthcare by identifying needs and provide information and referrals
    ● Offering health care system navigation services to the target population by facilitating enrollment in ACA (Obamacare) healthcare coverage, Medicaid, Medicare, Medical Access Program, or other healthcare options
    ● Connecting patients with health literacy resources and help them make informed healthcare decisions
    ● Connect patients with limited English proficiency to medical interpretation and translation services
    ● Demonstrating empathy, compassion and an understanding of the cultural and socioeconomic backgrounds to establish effective, trusting working relationships with patients and family caregivers

    Administrative Tasks
    ● Managing and tracking a large amount of information, such as patient profiles, appointments and interactions in a systematic and organized manner
    ● Meeting with the Program Manager regularly to ensure effective implementation of the program and track progress
    ● Maintaining a knowledge base of available resources, both in-house and external
    ● Maintaining patient privacy in compliance with HIPAA federal guidelines and AACHI policies

    Community Building
    ● Working with the Program Manager to develop a plan for targeted outreach, and potentially new strategies to engage clients
    ● Building close relationships with community and social service organizations to identify community needs, planning and conducting community outreach and engagement to bring all stakeholders together
    ● Providing advocacy support for issues such as patient rights, matters of privacy, confidentiality or informed consent, patient representation, awareness building, support and education of patients

    Qualifications:
    ● Bilingual English & Vietnamese proficiency required; must be proficient in reading, writing and speaking in both languages
    ● Must be able to effectively communicate (written & oral) and work well with a variety of stakeholders from different cultures
    ● Minimum high school diploma or its equivalent required, Bachelor’s degree in social sciences, public health, nursing, social work or other related discipline preferred; experience may be substituted for education
    ● Proficiency in Microsoft Office suite including Word, Excel, Outlook, and PowerPoint required
    ● Must have driver’s license and regular access to a car
    ● Must have flexible work schedule to meet community needs, including availability some evenings and weekends
    ● Must be willing to undergo a criminal background check
    ● Must be authorized to work in the United States
